Our local authority trained support planners work with individuals with a range of disabilities who are eligible for social care support, or who have a self-funded budget for social care.
We will discuss with you and help you to work through the seven ‘In Control’ steps:
- What is important to you?
- What do you want to change?
- How will you arrange your support?
- How will you spend your money?
- How will you manage your support?
- How will you stay in control?
- What will you do next?
We will work with you to develop a meaningful support plan which will empower you to be more in control of your life. Your plan will reflect your support and care needs, enabling you to be healthy, safe and well.
We have knowledge of relevant legislation, policies and procedures, knowledge of Self Directed Support and the ability to assist Service Users through the Self Directed Support process.
We have skills and the ability to create a meaningful person-centred support plan for you, working within your budget.
